Lower Back Pain in KL (Sakit Belakang Bawah) and the Link to Your Feet
Lower back pain (sakit belakang bawah) is one of the most common reasons Malaysians end up limiting how much they walk, stand, or exercise. Most people head straight to a chiropractor, physiotherapist, or GP and often that's exactly right. But there's one part of the picture that's easy to miss: your feet. The way you stand and walk sends force all the way up your legs to your pelvis and lower back, so when something's off at ground level, your back can be the part that complains.
Quick answer: A podiatrist does not treat your spine. But when your feet and gait are part of the problem like flat feet (kaki rata), overpronation, a leg-length difference, or worn-out, unsupportive shoes - correcting how your feet carry you can take strain off your lower back. It's not a replacement for physiotherapy or chiropractic care; it works alongside them, addressing the foot side of the chain.

The Foot–Back Connection / Hubungan Antara Kaki dan Belakang
Your body moves as one connected chain, from the ground up. Podiatrists call this the kinetic chain - kaki, buku lali, lutut, pinggul, dan belakang bawah semuanya berkait. When your feet don't support you well, the rest of the chain compensates:
Flat feet or overpronation (kaki rata): when the arch collapses inward with each step, the shin, thigh, and pelvis rotate slightly inward too. Over thousands of steps a day, that can tilt the pelvis and load the lower back unevenly.
A leg-length difference: even a small difference can tilt the pelvis and, over time, contribute to one-sided back ache.
Poor foot support or worn shoes: flat, unsupportive shoes and long hours on hard tiled floors mean your back absorbs shock your feet should have.

Important and honest: not all lower back pain comes from the feet. Plenty of back pain is muscular, disc-related, or spine-related and has nothing to do with your gait. That's exactly why an assessment matters. It tells you whether your feet are actually part of your problem, or not.

This is the question most people have, so here's the honest version. These roles overlap and often work best together:
A chiropractor focuses on spinal alignment and joint manipulation.
A physiotherapist focuses on rehabilitation, strengthening, and movement.
A podiatrist (that's us) focuses specifically on your feet, gait, and how the lower limb loads the ground and can prescribe orthotics to correct the foot side.

When to See a Podiatrist and When to Seek a Doctor First
Consider a podiatric biomechanical assessment if your lower back pain is ongoing, comes back whenever you're on your feet a lot, or comes together with foot, knee, or shin pain which are signs the whole chain may be involved.
But see a doctor urgently not a podiatrist if you have any of these red flags: sudden severe back pain after a fall or injury, numbness or weakness spreading down the leg, or any loss of bladder or bowel control or numbness around the groin/saddle area.
